Class Waiver & Liability Form

TERMS OF AGREEMENT

I agree that I will comply with all of Arts Iowa City's terms, signs, rules, and written and verbal instructions of ARTS IOWA CITY’S staff, contractors, and/or volunteers as conditions for entry and participation into any class or workshop of Arts Iowa City (“AIC”).

LIABILITY WAIVER

I, on behalf of myself, my heirs, assigns, representatives, and next of kin, agree to hold harmless, release, waive and indemnify AIC, its members, employees, contractors, volunteers, agents, successors and assigns from (A) any and all injuries, illnesses, medical conditions, claims, liabilities, damages, expenses or judgments from that derive from or related to my participation in AIC's programming and/or activities, including but not limited to the use of equipment, and participation in a class, program or event offered at AIC; and (B) any cost, fees, or expenses, including but not limited to attorney’s fees and costs, related to any and all injuries, illnesses, medical conditions, claims, liabilities, damages, expenses, or judgments arising from or related to the aforementioned participation in AIC’s programming.

I CERTIFY the information that I have entered herein to be accurate and true. I am at least 18 years old and I have read and agree to the terms of this agreement. Participants 14-17 years old may attend if accompanied by a parent or adult gaurdian.
